The Clinical Pharmacist supports member-centric clinical care program for Aetna Medicaid. The Clinical Pharmacist performs retrospective drug utilization reviews. They distribute clinical drug information and educate providers, staff, members/care givers regarding appropriate use of pharmaceutical treatment regimens. This individual will be responsible for performing pharmacy review and making evidence-based recommendations regarding Medicaid members, identifying opportunities within the health plan for integration of quality measures in current case management or other clinical programs, communicating to providers and members on quality indicators for Medicaid programs, providing clinical pharmacy support to programs targeted toward the Medicaid membership, and actively participating and collaborating as part of the multidisciplinary team to improve member's medication regimen and overall health. Additional duties may be required.

  • Perform pharmacy review and make evidence-based recommendations regarding Medicaid members.
  • Identify opportunities within the health plan for integration of quality measures in current case management or other clinical programs.
  • Communicate to providers and members on quality indicators for Medicaid programs.
  • Provide clinical pharmacy support to programs targeted toward the Medicaid membership.
  • Actively participate and collaborate as part of the multidisciplinary team to improve member's medication regimen and overall health.
  • Perform other duties as required.
  • Current unrestricted state pharmacist license in the State of Oklahoma, or ability and willingness to obtain within 90 days of hire.
  • Live in and be licensed in a state that allows clinical services to be performed in a work from home position.
  • 3+ years in a Clinical Pharmacist Role which must include at least one of the following: Rounding and making therapy recommendations as an active part of a multi-disciplinary care team; Independently managing patient therapy under a collaborative practice agreement; One year of a Pharmacy Practice Residency and/or At least 6 months in a current Medicaid Managed Care Clinical Pharmacy Role focused on Stars or HEDIS measures.
  • Established experience in functions beyond fundamental dispensing and order-processing activities such as optimization of medication selection, dosing, and monitoring.
  • Experience with population health and/or disease state management.
  • Ability to research and analyze medication literature including drug information, disease states, and clinical practice guidelines to identify gaps in care and create/communicate a recommendation plan to providers.
