The Clinical Pharmacist I-Retail supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills. Requirements

  • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree
  • Experience as a pharmacist in a retail and/or hospital environment is preferred
  • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required
  • CT Pharmacist Licensure required, active, and without restrictions
  • BLS (Basic Life Support) certification and immunization certification required
  • MTM (Medication Therapy Management) certification preferred
  • Demonstrate basic knowledge of pharmacology and therapeutics and understand pharmacy systems of drug distribution and formulary management
  • Excellent verbal and written communication skills and the ability to work with others are required
  • Must have expertise in patient management

Nice To Haves

  • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required
  • MTM (Medication Therapy Management) certification preferred

Responsibilities

  • Demonstrate and promote CHAMP behaviors
  • Provide excellent customer service to patients and other healthcare team members, with attention to the needs of all staff, with positive feedback from staff and co-workers
  • Provide medication counseling to patients and families, as applicable to the work area
  • Demonstrate effective oral and written communication skills
  • Demonstrate ability to escalate issues to management and/or collaborating prescribers as appropriate
  • Conduct timely, accurate medication order review that considers all patient characteristics including lab values and applicable vital signs
  • Provide medication therapy monitoring for assigned patients, adjusting medication therapy based on patient factors
  • Develop an effective interdisciplinary relationship with the health care team
  • Function as a source of medication information for the health care team
  • Participate in patient-centered initiatives
  • Ensure clinical continuity where applicable to the work area
  • Demonstrate competency in pharmacy automation and technology as applicable to the work area
  • Oversee non-sterile compounding and medication re-packaging to ensure compliance with regulatory statutes (e.g. USP Chapters), as applicable to the work area
  • Oversee medication distribution/dispensing process in the work area, to ensure safe, effective, timely medication distribution and prescription dispensing
  • Adhere to all departmental policies, procedures, protocols, and guidelines
  • Adhere to appropriate state and federal regulations
  • Report medication events and near misses
  • Demonstrate HRO principles and conduct in all interactions with patients and staff
  • Participate in quality assurance activities
  • Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members
  • Participate in professional development activities
  • Attend departmental staff meetings
  • Oversee pharmacy staff as necessary in the work area
  • Contribute to departmental cost-reduction/revenue generation
  • Participate in pharmacist-driven cost management protocols where applicable
  • Provide cost-effective clinical care
