A care giver in this role will be based at Fairview Hospital but cover PTO and call offs on site to support primarily west region oncology infusion centers. Cleveland Clinic's inpatient pharmacy areas support distributive and clinical pharmacy services provided by staff pharmacists, clinical pharmacy specialists and pharmacy technicians. Pharmacists are responsible for knowing the purposes, effects, and side effects of various drugs. They should be able to tell from looking at a patient's profile whether or not it is safe to combine new medications with the medications that the patient is already taking. Modern drugs have the potential to significantly improve patient's quality of life, however, they must be taken correctly, with the proper frequency, and in the appropriate dosage. A caregiver in this position works on a hybrid regional schedule, Monday through Friday 7:30 pm - 4:00 pm or 8:00 am - 4:30 pm. This caregiver will provide coverage on site or float throughout the regional oncology and infusion locations.

  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate, and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
  • PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
  • Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
  • Current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days.
  • Current Advanced Cardiovascular Life Support (ACLS) and/or Pediatric Advanced Life Support (PALS) certification through American Heart Association (AHA).
  • Completion of a post-graduate year 1 residency (PGY1), fellowship or 2 years of relevant post-graduate pharmacy experience.
  • Post-graduate year 2 residency (PGY2) or relevant experience for Clinical Pharmacists hired in a specialized pharmacy practice area (i.e. Critical Care, Oncology).
  • Oncology infusion experience preferred.
  • Understand sterile product and compounding practices.
