The Clinical Pharmacist, Population Health, is responsible for identifying opportunities to improve quality and outcomes of care and managing pharmacy expenditures at the population health levels. This individual will provide consultation to and review of the medication regimen of the high-risk populations/patients identified within the Hackensack Meridian Health CIN population. This includes education across the health care system and implementing cost-saving initiatives. The clinical pharmacists will provide innovative pharmacy services to patients served by Population Health. Specifically, in preventing initial inpatient admissions, preventing hospital readmissions, and decreasing inpatient and outpatient spending. The clinical pharmacist shall serve as a resource for care coordination teams and CIN providers.
