About The Position

The Clinical Pharmacist - Part Time position at Option Care Health involves providing safe and appropriate pharmacy services to patients in home care settings. This entry-level role requires coordination with nursing staff, monitoring patient progress, and educating patients and caregivers about medication use. The pharmacist will design and evaluate patient-specific pharmacy care plans, ensuring compliance with regulatory standards and Option Care policies.

Requirements

  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • At least one year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding, immunizations, or completion of an ASHP accredited Pharmacy Residency Program.

Nice To Haves

  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Experience in home care, hospital, or other clean room settings.

Responsibilities

  • Determine the suitability of individual patients for home care by collecting and organizing patient-specific information.
  • Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for home care patients.
  • Modify pharmacy care plans based on evaluation of monitoring data and patient-specific information.
  • Provide medication-use education to Option Care staff, other professionals, and home care patients or their caregivers.
  • Ensure continuity of pharmaceutical care to and from the home and other patient-care settings.
  • Prepare and dispense medications following appropriate techniques and safety practices.
  • Manage the use, maintenance, and troubleshooting of medication administration equipment.
  • Respond to requests for drug information from healthcare providers and home care patients.
  • Manage the use of investigational drug products according to established regulations and policies.
  • Maintain confidentiality of patient and proprietary information.
