About The Position

The PACE Pharmacist supports the Program of All-Inclusive Care for the Elderly (PACE) by ensuring safe, effective, and cost-effective medication management for PACE participants. The Pharmacist serves as a core clinical resource to the Interdisciplinary Team (IDT), providing comprehensive medication reviews, participant and caregiver education, formulary management, and regulatory compliance in accordance with CMS, state, and PACE program requirements.

Requirements

  • Doctor of Pharmacy (PharmD) required.
  • Minimum of 3 years of clinical pharmacy experience.
  • Active NYS Pharmacist licensure required.

Nice To Haves

  • Board Certification in Ambulatory Care Pharmacy preferred.
  • Prior experience in geriatrics, long-term care, managed care, or ambulatory care
  • Experience in a PACE program or similar integrated care model
  • Board Certification in Geriatric Pharmacy (BCGP) or Pharmacotherapy (BCPS)
  • Knowledge of CMS regulations and Medicare/Medicaid requirements
  • Strong interdisciplinary collaboration and communication skills
  • Experience with electronic health records (EHR) and medication management systems
  • Familiarity with value-based care models and population health

Responsibilities

  • Conduct comprehensive medication reviews for all PACE participants at enrollment, quarterly, annually, and with any significant change in condition.
  • Identify, resolve, and prevent medication-related problems, including polypharmacy, adverse drug reactions, duplications, and drug-drug or drug-disease interactions.
  • Provide medication therapy management (MTM) tailored to geriatric and medically complex populations.
  • Participate in Interdisciplinary Team (IDT) meetings, offering pharmacy expertise to inform individualized care plans.
  • Collaborate with providers to optimize pharmacologic regimens and support deprescribing initiatives when appropriate.
  • Monitor therapeutic outcomes, medication adherence, and participant safety.
  • Support the PACE program’s value-based care model by optimizing medication review and reconciliation workflows to improve participant outcomes, enhance quality of life, and reduce avoidable costs.
  • Oversee medication procurement, distribution, storage, and disposal in compliance with federal and state regulations.
  • Ensure adherence to the PACE formulary and contribute to formulary development and review.
  • Coordinate with contracted pharmacies, long-term care pharmacies, and suppliers to ensure continuity of care.
  • Facilitate transitions of care, including hospital admissions, discharges, and changes in care settings, ensuring accurate medication reconciliation across all settings.
  • Educate participants, caregivers, and PACE staff on medication use, side effects, and adherence strategies.
  • Provide staff training on medication administration, high-risk medications, and geriatric pharmacotherapy best practices.
  • Serve as a clinical resource for IDT members regarding pharmacology and medication safety.
  • Ensure pharmacy services comply with CMS PACE regulations, state pharmacy laws, and accreditation standards.
  • Participate in quality improvement initiatives, medication safety programs, and audits.
  • Assist in the development and maintenance of policies and procedures related to pharmacy services and medication management.
  • Track and report pharmacy-related quality metrics and outcomes to support continuous improvement.
