The Clinical Pharmacist provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers.
Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians on drug therapy issues.
Coordinates the activities of assigned pharmacy technicians in the preparation, labeling, and delivery of medications.
Ensures compliance with federal, state, and local laws and regulations regarding drug dispensing.
Mentors and trains others in training including practical experiences.
Maintains liaison relationships with medical and nursing staff.
Conducts and evaluates medication histories and assesses compliance.
Individualizes medication regimens based on pharmacodynamic and pharmacokinetic principles.
Extracts pertinent information from patient sources for medical management plans.
Role models communication of the organization's mission, vision, values, and strategic plan.
Supports Lean principles of continuous improvement.
Provides and maintains a safe environment for caregivers, patients, and guests.
Conducts all activities with professionalism and confidentiality.
Delivers customer service and/or patient care in a timely, efficient, and accurate manner.
May perform additional duties of similar complexity as required.
Bachelor of Science degree in Pharmacy.
Current registration by the Oregon Board of Pharmacy.
AHA Basic Life Support for Healthcare Provider certification (not required if working exclusively in Specialty Pharmacy).
PharmD.
Completion of an ASHP accredited PGY-1 residency training.
Any certification through the Board of Pharmacy Specialties (BPS).
Six (6) months or more clinical pharmacy experience.
