The Clinical Pharmacist provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers.

  • Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians on drug therapy issues.
  • Coordinates the activities of assigned pharmacy technicians in the preparation, labeling, and delivery of medications.
  • Ensures compliance with federal, state, and local laws and regulations regarding drug dispensing.
  • Mentors and trains others in training including practical experiences.
  • Maintains liaison relationships with medical and nursing staff.
  • Conducts and evaluates medication histories and assesses compliance.
  • Individualizes medication regimens based on pharmacodynamic and pharmacokinetic variations.
  • Extracts pertinent information from patient/other sources for the patient's medical management plan.
  • Role models two-way communication of the St. Charles Health System mission, vision, values, and strategic plan.
  • Supports Lean principles of continuous improvement.
  • Provides and maintains a safe environment for caregivers, patients, and guests.
  • Delivers customer service and/or patient care in a timely, efficient, and accurate manner.
  • May perform additional duties of similar complexity within the organization.
  • Bachelor of Science degree in Pharmacy.
  • Current registration by the Oregon Board of Pharmacy.
  • AHA Basic Life Support for Healthcare Provider certification.
  • PharmD.
  • Completion of an ASHP accredited PGY-1 residency training.
  • Any certification through the Board of Pharmacy Specialties (BPS).
  • Six (6) months or more clinical pharmacy experience.
  • Competitive salary range of $69.98 - $90.99.
  • Regular caregiver type with a scheduled weekly hour of 36.
