About The Position

Work Schedule : 100% FTE, day shift. Monday – Friday; 8:00 AM – 4:30 PM. This is a hybrid role, with occasional on-site requirements at University Hospital in Madison, WI and the Administrative Office Building in Middleton, WI. We are seeking a Clinical Pharmacist to: Develop and launch pharmacy services with collaboration from the pharmacy manager and clinical team, with the goal of enhancing patient access, optimizing medication access, and improving patient outcomes. Deliver medication therapy management services to optimize patient outcomes and ensure safe, effective, and cost-efficient drug therapy. Conduct telehealth consultations with headache patients referred by primary care providers and neurology specialists. Provide guidance and support for medication access and affordability programs to reduce barriers to care. The ideal candidate for this role would have a Pharmacy Residency focused in ambulatory care. Requirements

• Bachelor's Degree in Pharmacy or a PharmD degree Required
• 1 year of clinical patient care and orders management experience Required
• Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
• Basic Life Support (BLS) within 90 Days Required

Nice To Haves

• Completion of an ASHP Accredited Residency training program Preferred
• 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred
• Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
