The pharmacist serves as part of the healthcare team by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures.

Requirements

  • Bachelor's Degree in Pharmacy or Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification
  • 1 year of experience (Postgraduate year 1 residency or fellowship OR a minimum of 2 years of experience as a registered pharmacist in health system pharmacy)
  • Current Pharmacist licensure in the state of Virginia completed within 30 days of start date by either examination or reciprocity
  • Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years of start date as applicable to the role (excludes PRN team members)
  • American Heart Association (AHA) Health Care Provider BLS certification
  • American Heart Association (AHA) Health Care Provider ACLS certification within six months of hire
  • Ability to drive/travel to multiple facilities/locations as needed
  • Ability to work as part of a high performing, change ready and resilient team
  • Other duties as outlined in UVA Health Northern Virginia Culpeper and facility specific policies and procedures

Responsibilities

  • Supporting authentic personalized relationships with patients and their chosen support system
  • Collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health
  • Accepting responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures
  • Providing optimal medication management and promotion of medication safety
  • Performing patient assessments
  • Participating in direct patient care activities and rounding opportunities where available
  • Monitoring therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions
  • Reviewing and acting upon clinical decision support surveillance
  • Completing organizational and/or departmental required documentation
  • Prescribing and/or administering medications as allowed by law and scope of practice
  • Maintaining up-to-date, evidence-based knowledge required for services provided
  • Ensuring timely and consistent communication
  • Ensuring timely responses to service line requests and inquiries
  • Participating in patient education and wellness-based initiatives as driven by scope of practice
  • Checking and controlling quality and accuracy of work performed by pharmacy team members
  • Prioritizing workflow ensuring all services are performed in a timely manner
  • Maintaining and following formulary approvals, interchanges, stocked medications, and non-formulary process
  • Monitoring and enhancing compliance with clinical programs
  • Developing and delivering effective education to pharmacists and other healthcare providers
  • Serving as a preceptor to residents and students
  • Participating in the onboarding of pharmacists and other team members
  • Providing ongoing feedback to fellow pharmacists to achieve desired outcomes
  • Serving as a mentor to pharmacy team
  • Performing additional duties as assigned
