A Clinical Pharmacist contributes to AHF by being assigned to more than one location, that is based on a scheduled need of the Practice Sites, in the geographical area of the individuals primary residence. Expanding primary care functions of pharmacist such as providing individualized health promotion and disease prevention, including administration of vaccines, long acting HIV treatments, diabetes monitoring, nicotine reduction programs and other subject matters, in collaboration with other AHF Providers. A Clinical Pharmacist enhances the reputation of AHF by improving the outcomes for the patients served by AHF, enhance the services that AHF provides, expand the opportunities for professional growth and satisfaction and assist with the on-going development of the AHF Healthcare System. You will be a passionate advocate to our top initiatives. To be successful as a Clinical Pharmacist it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all you do. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Clinical Pharmacist will work as part of a close knit team to provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessations programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority. Includes prescriptive authority where this is legally and organizationally authorized such as PrEP and PEP therapies.
