The clinical pharmacist is responsible for all aspects of pharmaceutical care in the adult acute care population. The pharmacist will provide services in the intermediate care areas, including but not limited to, internal medicine, neurology, and oncology patient populations. In addition, this position may include training to other inpatient clinical areas as staffing needs dictate. This position will have rotating shifts including weekdays, weekends, and evenings. Clinical pharmacists at UIHC are involved in all aspects of patient care such as therapeutic drug monitoring, medication reconciliation, medication education, and precepting pharmacy learners. This role will support the highest level of safe and effective patient care via thorough clinical review and verification of medication orders. In addition, all members of the pharmacy team collaborate daily with members of the interdisciplinary team, including licensed independent practitioners, medical students and residents, nurses, social workers, and care coordinators.
