Clinical Pharmacists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel. In these clinical roles, Clinical Pharmacists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy Students and pharmacy residents.   • Delivers direct patient care and clinical practice, including decentralized and service-based programs
  • Provides pharmaceutical services throughout the medical center
  • Performs alternate leader responsibilities/duties as assigned in the absence of the pharmacy manager (leader).
  • Leads assigned patient service lines, clinical areas, and therapeutic programs
  • Participates in pharmacy operations and medication dispensing
  • Oversee pharmacy operations and technician practice/activities
  • Facilitates experiential education and practice advancement
  • Utilizes effective communication to provide excellent customer services.
  • Actively participates in performance improvement activities as it relates to job duties.
  • Demonstrates effective and culturally appropriate communications.
  • Performs all other duties as requested by direct supervisor, department or senior leadership
  • When necessary, may assist in instructing didactic classroom instruction for the University of Toledo
  • All other duties as assigned.
  • PharmD degree from an ACPE-accredited College of Pharmacy or 5 years clinical experience preferred.
  • Formal clinical training through a PGY1 and/or PGY2 residency, fellowship, or 3 years of applicable clinical practice (setting dependent on specialty) preferred.
  • Must hold current and unrestricted Ohio Pharmacist License.
  • BLS and ACLS Certification.
  • This position requires a flexible schedule due to operational needs of the department.
  • As determined by the Pharmacy Manger, for those clinical pharmacist positions required to work under a consult agreement, credentialing and board certification is required within 3 years of employment.
  • Must become an ASHP PGY1 residency preceptor upon eligibility.
  • Experience in providing written and oral drug information with excellent written and oral communication skills.
  • Analyze data, interpret policies, procedures and regulations, develop appropriate conclusions, and prepare reports.
  • Knowledge of contemporary hospital/clinical practice and service.
  • Significant knowledge and expertise in advanced pharmacy practice, and specialty evidence-based medicine in area appropriate for clinical assignments.
  • Self-directed, motivated and capable of leading supportive personnel.
  • Possess the ability to resolve conflicts and respond appropriately to complaints.
  • Able to work well with the public and other health professionals.
  • Able to train, evaluate and supervise professional and technical personnel and staff.
  • Demonstrate and utilize effective interpersonal and culturally appropriate communications.
  • Visual acuity of 20/20 or corrected to 20/20 including color vision when applicable
  • Ability to hear (with or without aides)
  • Ability to orally communicate
  • Good manual dexterity
  • Ability to push, pull, roll, and transfer/lift 50 pounds occasionally
  • Ability to stand, walk constantly
  • Ability to bend, reach, stoop, kneel occasionally
  • Board certification preferred.
