About The Position

Sinai Chicago is seeking a patient-centered, detail-oriented General Medicine Pharmacist to join our inpatient pharmacy team in a full-time capacity. This role provides comprehensive pharmaceutical care to general medicine units, including order verification, therapeutic drug monitoring, and interprofessional collaboration with physicians, nurses, and care teams. As a pharmacist in a teaching hospital, you will also play a key role in educating students and residents, supporting safe, evidence-based medication use, and ensuring regulatory compliance throughout daily clinical operations.

Requirements

  • Doctor of Pharmacy (PharmD) from an ACPE-accredited college of pharmacy.
  • Current Illinois Pharmacist License in good standing.
  • Hospital pharmacy experience required (completion of a PGY1 residency preferred).
  • Strong clinical knowledge in internal medicine, pharmacokinetics, and evidence-based practice.
  • Excellent communication and critical thinking skills.
  • Ability to work collaboratively in a team-based, fast-paced environment.

Nice To Haves

  • Experience with EPIC EMR and automated dispensing systems (e.g., Omnicell) preferred.

Responsibilities

  • Verify, evaluate, and process medication orders in a timely and accurate manner.
  • Monitor drug therapy for appropriate indication, dosing, interactions, renal/hepatic function, and cost-effectiveness.
  • Provide drug information and therapeutic recommendations to providers and nursing staff.
  • Monitor and adjust therapies for anticoagulation, vancomycin, aminoglycosides, and other high-risk medications.
  • Participate in patient care rounds as needed.
  • Support discharge medication counseling and transitions of care when appropriate.
  • Communicate actively with the medical team regarding patient-specific drug therapy plans.
  • Support nursing workflows through clarification of medication orders, IV compatibility, and safe administration practices.
  • Coordinate with case management, dietitians, and providers to ensure continuity and safety of care.
  • Ensure compliance with hospital policies, state/federal pharmacy laws, and Joint Commission standards.
  • Participate in quality improvement initiatives, including medication use evaluations and antimicrobial stewardship.
  • Document interventions, adverse drug events, and therapy recommendations accurately in the EMR.
  • Precept pharmacy students and PGY1 residents as part of the experiential education team.
  • Participate in the development and review of clinical protocols, order sets, and guidelines.
