Ensures that the pharmacy provides optimal clinical services on a daily basis, promoting desirable patient outcomes through safe and cost-effective drug therapies. Establishes and maintains productive collaborative relationships with practitioners and hospital staff. Supports all safety and regulatory initiatives undertaken by the hospital and department. Demonstrates the knowledge and skill necessary to identify and provide pharmaceutical care to the populations served at the Barton Healthcare System: including neonatal, pediatric, adult, geriatric, obstetric, chemically dependent, and rehabilitation patients. Participates in all aspects of hospital pharmacy services including but not limited to operations, distribution and clinical pharmacy activities.

  • Monitors drug therapy of all admitted and infusion center patients.
  • Collaborates with providers to optimize and address any issues with drug therapies.
  • Provides pharmacokinetic dosing and consultation for provider specified medication therapies according to hospital protocols and procedures.
  • Participates in multidisciplinary rounding with the health care team.
  • Performs patient counseling for inpatients on new therapies or discharge medications.
  • Participates in medication reviews including the addition and removal of medications to the hospital formulary.
  • Participates in full traumas and code blues to provide pharmaceutical support and expertise.
  • Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways.
  • Ensures that the pharmacy provides optimal distribution services on a daily basis.
  • Responsible for the safe and accurate distribution of drugs to inpatients, outpatients, and clinics.
  • Supervises and directs pharmacy support personnel in a professional manner.
  • Mentors new pharmacists, pharmacy technicians and pharmacy interns.
  • Establishes and maintains productive collaborative relationships with practitioners and hospital staff.
  • Completes annual mandatory education related to clinical pharmacy practice and compounding practices.
  • Performs monthly area inspections and collaborates with staff to ensure compliance with all relevant hospital policies.
  • Provides on call pharmaceutical support for clinically necessary situations when assigned.
  • Identifies and collaborates to correct EMR build issues related to medications.
  • Pharm.D. required.
  • Minimum 1 year experience in hospital/similar acute care pharmacy setting.
  • Valid and clear California pharmacist license.
  • Current BLS certification.
  • Prefer completion of PGY 1 or 2 clinical residency.
  • Prefer valid and clear Nevada pharmacist license.
  • Prefer ACLS and PALS certifications or ability to obtain.
