Capital Health Medical Group is made up of more than 250 physicians and other providers who offer primary and specialty care, as well as hospital-based services, to patients throughout the region. The Clinical Pharmacist promotes a safe and cost-effective medication management system for patients at Capital Health within his/her assigned practice area. This role involves providing education, information, and publications on pharmaceutical care and drug therapy, developing and supporting quality initiatives through data management and analysis, and collaborating with Pharmacy staff to effectively operate the department.

Responsibilities

  • Interprets, evaluates, and accurately processes drug orders for patients receiving care in assigned areas.
  • Identifies and resolves problems with medication orders or therapeutic regimens.
  • Provides concise, applicable, comprehensive, and timely responses to requests for drug information from patients and healthcare providers.
  • Communicates between shifts and with peers to ensure continuity of care and optimal delivery of pharmaceutical services.
  • Demonstrates ownership of and responsibility for welfare of patient by performing all necessary aspects of medication-use system.
  • Provides all clinical services and direct patient care services for assigned populations.
  • Ensures accurate and timely medication-specific information regarding a specific patient is communicated to those who need it.
  • Documents pharmaceutical care interventions in departmental reporting system.
  • Establishes collaborative professional pharmacist-patient relationships.
  • Identifies opportunities for improvement of medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs.
  • Demonstrates teamwork within Pharmacy Department and participate in group approach to patient care.
  • Serves as an internal and external liaison for goals and vision of Pharmacy Department services.
  • Represents Pharmacy Department at hospital meetings and events as needed.
  • Serves as a preceptor for pharmacy residency and member of Residency Advisory Committee.
  • Designs and implements quality improvement changes to organization's medication-use system.
  • Provides effective evidence-based medication and practice-related education to healthcare professionals.
  • Participates in orientation, education, and mentoring of new staff.
  • Contributes to total body of knowledge in pharmacy practice through presentations, publications, and professional affiliation activities.

Requirements

  • Graduate of an ACPE-accredited school of pharmacy.
  • Doctor of Pharmacy (Pharm D) or equivalent advanced degree.
  • Board of Pharmacy Specialties (BPS) certification preferred.
  • PGY1 Pharmacy Residency or three years equivalent experience preferred.
  • AHA ACLS-Adv Cardiac Life Supp, AHA BLS - Healthcare Provider, Pharmacist - Registered.
  • 2 hours of Stroke related Continuing Education contact hours annually.
  • Possesses excellent organizational, interpersonal, conflict resolution, verbal, and written communication skills.
  • Ability to effectively manage multiple projects and responsibilities simultaneously.
  • Ability to respond quickly in a fast paced environment.
  • Intermediate to advanced computer skills.
  • ASHP-accredited pharmacy residency training preferred.
