About The Position

The Cystic Fibrosis (CF) Clinical Pharmacist position will provide pharmacotherapeutic support to the pediatric and adult patients living with cystic fibrosis in the ambulatory care setting. The CF clinical pharmacist will ensure optimal medication use, support adherence, and actively contribute to individualized care planning while working alongside pulmonologists, nurses, dietitians, respiratory therapists, and social workers as part of an accredited Cystic Fibrosis Care Center. This pharmacist will also participate in multidisciplinary committees and serve as a preceptor in the Atlantic Health System PGY1 Pharmacy Residency Program.

Requirements

  • Must be a graduate of an accredited school of Pharmacy with PharmD degree required.
  • Completion of a PGY1 residency and PGY2 residency are preferred (ambulatory care, internal medicine, pharmacotherapy, and infectious diseases, etc).
  • Board certification in pharmacotherapy (BCPS) is preferred.
  • Current NJ Board of Pharmacy license required.
  • Minimum of 1-year experience in a hospital practice setting.
  • Experience in research activities also preferred.

Responsibilities

  • Provide comprehensive pharmaceutical care to patients with cystic fibrosis, including review and recommend optimal, evidence-based medication therapy.
  • Provide drug information and pharmacokinetic consultations as required.
  • Document any changes in patient and pharmacy records and inform others of medication changes.
  • Prevent, detect, monitor, document, and report adverse drug reactions and medications errors.
  • Evaluate and monitor complex medication regimens, including CFTR modulators, antibiotics, pancreatic enzymes, and other disease-specific therapies.
  • Coordinate transition-of-care services and medication access, including medication histories, medication education, prior authorizations, specialty pharmacy navigation, and formulary agent selection.
  • Develop and implement CF-related protocols, guidelines, and quality improvement initiatives.
  • Collaborate with inpatient providers and pharmacists to ensure discharge medication access and monitoring as necessary.
  • Serve as a preceptor and mentor for pharmacists, pharmacy residents, and students.
  • Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with the goal of publication and/or presentation at local and/or national meetings.
  • Attend annual North American Cystic Fibrosis Conference with funding support.
