About The Position

The Clinical Pharmacist, Care Management will perform comprehensive and targeted medication management in high risk plan members to identify, resolve, and prevent medication-related problems to improve care outcomes. The role involves providing clinical recommendations, conducting medication reviews, engaging in telephonic consultations, and collaborating with the patient care team. The pharmacist will also provide medication education, navigate benefits, and maintain knowledge of clinical trends.

Requirements

  • Pharm.D. or equivalent degree from an accredited school of pharmacy in the USA required.
  • Current pharmacist licensure in the State of California.
  • Dynamic and confident person with strong clinical skills.
  • Strong communication skills and the ability to build positive relationships.
  • Must be highly detail oriented, motivated, and independent.
  • Comprehensive knowledge of medical terminology related to drug usage.
  • Ability to assess and apply current pharmacological principles in a clinical setting.
  • Ability to extract pertinent information from patients and relate it to the care plan.
  • Knowledge of laws, regulations, and standards relative to pharmacy practice.

Nice To Haves

  • Completion of a PGY1 and/or PGY2 residency or equivalent clinical practice experience preferred.
  • Board Certification preferred (Geriatric, AmCare, or Pharmacotherapy).
  • Certified Diabetes Care and Education Specialist (CDCES)/Certified Diabetes Educator (CDE) preferred.
  • Managed care experience with knowledge of CMS Part D regulations preferred.
  • Experience in palliative care preferred.
  • Bilingual in English/Spanish strongly preferred.

Responsibilities

  • Provide clinical recommendations pertaining to gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution.
  • Complete comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling.
  • Conduct engaging and comprehensive telephonic medication consultations with members.
  • Utilize prescription and medical claim data to generate clinical recommendations for medication management.
  • Complete pharmacotherapy assessment of patients' claims, past medical history, and disease states.
  • Collaborate with all members of the patient care team by actively communicating and reporting pertinent patient care information.
  • Provide medication education to patients to improve medication adherence.
  • Monitor complex comorbidities such as DM, HTN, hyperlipidemia, and others.
  • See the whole person, taking into account Social Determinants of Health.
  • Research and maintain knowledge of current and emerging clinical trends.
  • Actively participate in various clinical departmental meetings and forums.
  • Maintain competency skills to identify medication safety concerns in the age-related population.
  • Serve as preceptor to pharmacy students and residents as assigned.
