This role focuses on providing pharmacy related services in the Cardiology Clinic at Tufts Medical Center, including medication counseling, titration, and follow-up for the Advanced Heart Failure, General Cardiology, and Cardiac Subspecialty clinics. It is a professional individual contributor role that may direct the work of other lower-level professionals or manage processes and programs. The position requires specialized knowledge and skills, typically acquired through advanced education in Cardiology and Specialty Pharmacy, and applies practical knowledge obtained through advanced education and work experience. The Clinical Pharmacist works independently with general supervision, addressing difficult but typically not complex problems, and may influence others within the job area. The role is responsible for the day-to-day operation of the Cardiology Clinics, directing and coordinating activities of a pharmacy area, including direct supervision of professional and support staff. It ensures pharmacy practice aligns with department policies and procedures, promotes best pharmacy practices within state and federal laws, rules, and regulations, and identifies operational improvements for efficient, high-quality pharmaceutical care. Clinical pharmacists work closely with other health practitioners to review medication appropriateness, monitor drug therapies, and provide drug information, thereby improving patient health. The position provides leadership by facilitating the development, implementation, and maintenance of hospital and departmental systems that promote desirable patient outcomes, and educates students, interns, residents, nurses, and patients while participating in drug therapy research.
