About The Position

The Clinical Pharmacist at Monument Health's Cancer Care Institute is responsible for delivering comprehensive medication management to optimize patient outcomes. Requirements

• Bachelor's degree in Pharmacy.
• Pharmacist certification from the South Dakota Board of Pharmacy.
• Advanced Cardiac Life Support (ACLS) certification from the American Heart Association (AHA).
• Basic Life Support (BLS) certification from the American Heart Association (AHA).
• 3+ years of hospital experience.
• 3+ years of clinical experience.
• 1+ years of pharmaceutical experience.

Nice To Haves

• Doctorate in Pharmacy.
• Post-graduate Year 1 (PGY-1) certification from the American College of Clinical Pharmacy (ACCP).
• Pediatric Advanced Life Support (PALS) certification from the American Heart Association (AHA).

Responsibilities

• Identifies medication-related problems such as appropriateness of drug, dose, route, frequency, and duration.
• Monitors patient response to therapy.
• Provides accurate drug information to multi-disciplinary team members and educational information to patients and families.
• Manages pharmacy protocols including heparin, warfarin, pharmacokinetic, IV to PO, and therapeutic interchange.
• Participates in quality improvement projects.
• Leads development, implementation, and maintenance of pharmacy services in a specialty area under guidance from pharmacy leadership.
• Acts as a resource in area of specialty.
• Documents consults, pharmacist interventions, adverse drug reactions, and medication variances consistently.
