The Clinical Pharmacist supports the mission of the department, hospital, and health-system by preventing, identifying, and resolving medication use issues to assure patient safety, improve patient satisfaction, optimize patient outcomes, and minimize costs. The role involves working in both centralized and decentralized areas, performing all functions of a registered pharmacist including accurate and timely dispensing, clinical interactions with patients and the healthcare team, and technician supervision. This position is responsible for developing and providing clinical pharmacy services to the general patient population, supervising pharmacy technicians to ensure correct and timely medication dispensing and ensuring medication therapies are appropriate for patients based on a variety of factors. Additionally, the Clinical Pharmacist interprets prescriptions and medication orders, acts as a drug information resource, compounds and dispenses prescribed medications, and participates in patient care rounds and medication management. Responsibilities also include assisting with the daily operation of the pharmacy department, participating in performance improvement activities, educating staff, managing adverse drug events, participating in quality improvement initiatives, ensuring compliance with regulations, and engaging in continuous professional development. The role requires collaboration with healthcare teams to ensure the safe and effective use of medications and maintain efficient pharmacy operations.

  • Actively performs all pharmacist functions and participates in established standard work parameters.
  • Maintains operational expertise in and provides coverage of central pharmacy, IV room pharmacy, and/or satellite pharmacy areas.
  • Oversees and ensures appropriate utilization of medication automation, technology, and computer systems.
  • Accurately manages medication inventories and prepares and dispenses patient-specific medication doses.
  • Supervises technician and intern practice, prioritizes workload, organizes workflow, and checks accuracy of pharmacy technician activities.
  • Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems.
  • Proficiently verifies/enters medication orders and communicates with medical staff to resolve problem medication orders.
  • Provides patient-centered care services, including accurate, safe, and timely medication therapy.
  • Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults.
  • Facilitates medication safety practices, including identifying, mitigating, and reporting potential and actual adverse drug events.
  • Participates in the management of medical emergencies.
  • Actively participates in direct patient care programs and collaborates with healthcare providers and pharmacy staff.
  • Presents to assigned areas on time and ready to work to ensure continuity of care.
  • Participates in departmental meetings and educational activities as assigned.
