Learn more at HonorHealth.com. Job Summary The Ambulatory Care Clinical Pharmacist provides medication management through active participation on the healthcare team in the ambulatory setting. The Ambulatory Care Clinical Pharmacist contributes to the selection of individualized drug therapy, implementation of patient-specific medication regimens, laboratory monitoring, and provision of comprehensive medication education to maximize therapeutic benefit and minimize risks of adverse events. Population health management strategies are utilized to improve medication-related outcomes and reduce hospitalizations for at-risk populations. Conducts educational activities, applies principles of performance improvement, and optimizes care in a cost-effective manner. Provides direct patient care via collaborative drug therapy management in practice areas in which the Ambulatory Care Clinical Pharmacist maintains protocol certification and may include diabetes, anticoagulation, transitions of care, and other complex disease states. Assists with precepting pharmacy residents and students.
