About The Position

Novant Health is seeking a Clinical Pharmacist to join their ambulatory pharmacy team in Charlotte, NC. The role focuses on delivering optimal medication management and patient care, particularly in primary care settings. The Clinical Pharmacist will collaborate with healthcare providers, manage medication therapy, and contribute to quality improvement initiatives while supporting the educational development of the department.

Requirements

• Bachelor's Degree in Pharmacy required; Doctorate of Pharmacy preferred.
• Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in healthcare system pharmacy required.
• Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity required.
• Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years of start date as applicable to the role required.
• Direct patient care experience of 1-2 years in primary care setting with emphasis on diabetes, anticoagulation, lipids, HTN, obesity, osteoporosis patient management.
• Current BCPS or BCACP (board certification will be required post-employment for credentialing).
• Proficient bilingual skills in Spanish.

Responsibilities

• Partnership with NHMG providers and clinical staff to provide remarkable patient care.
• Medication therapy management and patient counseling.
• Promotes high quality, evidence-based, cost-effective pharmaceutical care.
• Precepting of residents (PGY1/PGY2) and students.
• Participation in pharmacy and NHMG committees and shared governance.
• Contributes to process improvement projects.
• Supports the educational and professional development of the department and system.
• Ability to communicate effectively both orally and in writing.
• Demonstrating proficiency with equipment and software.
