Position Summary: Clinical Pharmacist, Ambulatory Care Responsible for providing clinical pharmacy services in an outpatient and/or inpatient practice area, by providing drug therapy management and assessment for patients in the practice area. Provides support for established programs including, but not limited to, drug formulary, pharmacy education programs, and quality improvement and research relating to drug therapy. Description: This is a full-time (80 hours per pay-period), salaried, Monday thru Friday position for a Clinical Pharmacist, Ambulatory Care within the Trinity Health Medical Group primary care, PCMH offices (part of the Trinity Health Saint Mary's health-system in Grand Rapids, Michigan). Responsibilities include disease management within institution approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia, asthma, COPD, anticoagulation, others), referral-based Comprehensive Medication Review with recommendations to physicians, daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost as well as participating in team-based care within a multidisciplinary model. Additional activities include time in our Anticoagulation Management Service (approximately 1 day/week), and other multidisciplinary initiatives focused on particular high-risk patients or populations (e.g., depression, frail elderly) (approximately ½ day/week). Participation in the training of Trinity Health Grand Rapids PGY-1 & PGY-2 Pharmacy residents and PharmD students from the College of Pharmacy at Ferris State University is also expected as is participation in both medical group and/or health system committees.
