About the position

The Ambulatory Care Clinical Pharmacist will work with an interdisciplinary team to provide comprehensive disease state management and assist with clinical oversight of assigned patient population. The individual will promote coordinated care for these individuals throughout the health system to optimize quality and safety outcomes. The Ambulatory Care Clinical Pharmacist will be responsible for aiding in the education of healthcare professionals on pharmacotherapy management and serve as a resource and liaison for medication related issues as well as fostering and promoting the relationship between pharmacy and other healthcare professionals. Performs other duties as assigned.

Responsibilities

  • Performs disease state management under collaborative practice agreement for patients identified for and referred to the pharmacist led program within the ECU Health System ambulatory care clinics.
  • Collect and analyze information about the patient and is responsible for analyzing information in order to understand the patient's relevant medical/medication history and clinical status.
  • Assess collected information and formulate a medication therapy problem list consisting of the patients' active medical problems and medication therapy problems.
  • Develop and implement the care plan in collaboration with the health care team and the patient or caregiver.
  • Perform direct patient and family education to ensure comprehension and acknowledgment of therapy plans.
  • Collaborate with providers and staff to support prior authorization documentation and resolution of other identified barriers to adherence.
  • Provide ongoing follow-up and monitoring to optimize the care plan and identify and resolve medication therapy problems.
  • Attend and present as needed at clinic and system meetings related to ongoing work.
  • Provide periodic education to clinic and system providers regarding pertinent updates to guidelines, formularies, or other topics.
  • Provide education/clerkship experiences to pharmacy students and residents as requested.

Requirements

  • PharmD degree or equivalent (RPh) from accredited school of pharmacy is required.
  • Evidence of at least two years of relevant clinical pharmacy experience and/or formal residency training in a clinic-based, ambulatory practice.
  • PGY2 in Ambulatory Care may substitute for 2 years of work experience.
  • Must be licensed as a pharmacist by the NC Board of Pharmacy by start date.
  • American Heart Association Basic Life Support (BLS) certification required.
  • Obtains continuing education credits as required by the North Carolina Board of pharmacy.
