Requirements

  • High School graduate or GED equivalent.
  • Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist.
  • Current CPR certification that meets Company guidelines.
  • Knowledge of medical terminology.
  • Basic computer literacy, including EMR, iPad, and GPS experience.
  • Strong interpersonal communication and customer service skills.
  • Must maintain a valid driver's license and good driving record.

Nice To Haves

  • Certified Medical Assistant.
  • Six months of experience in patient care, preferably geriatric care.
  • Strong Phlebotomy skills.
  • Desire and ability to obtain MA certification with 180 days of hire.

Responsibilities

  • Assist Harmony Cares Primary Care Physician, Nurse Practitioners and Clinical Teams in the delivery of primary health care and patient care management in a residential setting.
  • Support Patient Care through Clinical Partnership with Nurse Practitioners, Physicians, and Care Management Teams.
  • Coordinate daily travel itinerary and transport clinicians to scheduled appointments.
  • Prepare patients for examination and treatment; may include preparing patients for minor procedures.
  • Secure patient consent forms, record patient care documentation accurately and in a timely manner.
  • Coordinate patient care as directed by Clinicians, company standards and policies.
  • Respect patient confidentiality at all times and treat patients with courtesy and respect.
  • Perform phlebotomy (venipuncture only), EKG or any other procedure as requested by Clinician.
  • Label, store and properly package specimens for lab delivery.
  • Assist in identifying and closure of quality gaps.
  • Document and update patient care plans, as well as SDoH (Social Determinants of Health) and other assessments within the patient chart or electronic medical record.
  • Maintain compliance with CLIA, OSHA, and HIPPAA guidelines.
  • Prepare and maintain supplies and equipment for treatment.
  • Ensure company vehicle is clean and proper periodic maintenance schedule is followed.
  • Attend required meetings and in-services.
