About The Position

This position provides in-person guidance and support to the providers, care teams, and affiliates in clinical integration initiatives that support the quadruple aim and allows the organization to thrive in a value-based health care model. The role involves maintaining a deep understanding of key drivers for each practice, translating data to insights, and collaboratively identifying actionable steps for improvement through a Plan-Do-Study-Act (PDSA) process improvement model.

Requirements

  • Current RN Licensure accredited by CCNE, ACEN, or CNEA (preferred).
  • BSN Bachelor's of Science Nursing or related Healthcare field (required).
  • 5 years of experience in a healthcare setting including ambulatory offices, with previous management or leadership experience in Population Health or Value-Based Contract work (required).

Nice To Haves

  • MSN Masters of Science Nursing, MBA, or Master’s degree in related Healthcare field (preferred).
  • Working knowledge of process improvement models (such as PDSA and A3) (preferred).
  • Experience in value-based arrangements and clinical reporting packages (such as Milliman or Tableau) (preferred).

Responsibilities

  • Completes in-person and virtual rounds in the practices to communicate status reports and provide insights to providers and practices.
  • Manages the review of clinical outcomes, patient panel management, and performance metrics for affiliates and employed physicians.
  • Partners with providers and care team members to implement evidence-based protocols and improvement initiatives.
  • Maintains a professional role model in practice transformation and engages regional stakeholders on practice improvement activities.
  • Conducts provider orientation and facilitates onboarding and offboarding of providers.
  • Supports and works closely with providers and care teams to implement process and quality improvement activities.
  • Collaborates with regional and practice staff to facilitate market implementation of network initiatives.
  • Conducts chart audits in support of annual quality composite audits.
  • Facilitates provider remediation tracking and activities in collaboration with network leadership.
  • Engages in relationship management to support ministry outcomes and delivery of care.
