About The Position

This is a dual-program leadership role for a passionate, experienced nurse leader to lead Hospice and Skilled Home Health programs. The Clinical Operations Manager will bring heart to end-of-life care and rigor to daily operations, impacting patients, families, and a clinical team through guidance, mentorship, and clinical excellence.

Requirements

  • Current, unrestricted Registered Nurse (RN) license in the state of practice
  • 3–5+ years of clinical experience in skilled home health and/or hospice
  • Demonstrated leadership or supervisory experience in a clinical setting
  • Strong knowledge of CMS CoPs, Medicare/Medicaid regulations, and clinical documentation standards
  • Proficiency with Electronic Medical Records (EMR) systems
  • Excellent communication, organizational, and problem-solving skills

Nice To Haves

  • BSN required; Master's degree a plus
  • Experience with OASIS documentation, QAPI programs, or regulatory survey preparation
  • CHPN (Certified Hospice and Palliative Nurse) or CRRN certification
  • Experience managing dual or multi-program clinical operations

Responsibilities

  • Oversee day-to-day clinical operations for both hospice and skilled home health programs
  • Provide hands-on guidance and support to nursing, therapy, aide, and social work staff
  • Monitor staffing, caseloads, and scheduling to ensure continuity and quality of care
  • Collaborate with the Administrator/Executive Director on strategic goals
  • Ensure compliance with CMS Conditions of Participation (CoPs) for both service lines
  • Lead survey readiness, internal audits, and corrective action plans
  • Audit clinical documentation - including OASIS assessments and hospice plan of care updates - for accuracy and regulatory compliance
  • Monitor quality metrics and drive performance improvement initiatives
  • Oversee clinical aspects of intake and admissions for hospice and home health referrals
  • Review referrals for medical necessity, eligibility, and payer requirements (Medicare, Medicaid, managed care)
  • Ensure timely, compliant Start of Care visits and smooth patient transitions
  • Lead interdisciplinary team (IDT) meetings and comprehensive care planning
  • Support symptom management, goals-of-care conversations, and end-of-life care delivery
  • Oversee bereavement and volunteer coordination programs
  • Oversee OASIS completion, accuracy, and submission timelines
  • Monitor visit utilization, episode management, and value-based care outcomes
  • Ensure therapy and nursing plans of care are individualized and outcome-driven
  • Recruit, onboard, mentor, and evaluate clinical staff across both programs
  • Oversee competency assessments, orientation, and ongoing clinical education
  • Foster a culture of accountability, teamwork, and continuous improvement
