About The Position

Always Best Care is a locally owned and operated home health and home care agency serving Oahu, providing compassionate, high-quality care to kupuna and families. As a new and growing Medicare-certified Home Health Agency, they are building strong referral relationships and expanding rapidly, focusing on reliable care while maintaining values of respect, accountability, and taking care of one another. The Clinical Operations Coordinator role is for a high-accountability, detail-driven operator to take ownership of scheduling and operational workflow. This is not a basic admin role; the coordinator will play a key part in supporting growth by ensuring patient visits are scheduled, completed, and supported without gaps. The position requires close coordination with clinicians, patients, and internal systems to keep operations running smoothly. It is ideal for someone who thrives in a fast-paced environment, enjoys solving problems in real-time, and takes pride in doing things the right way.

Nice To Haves

  • Experience in healthcare (home health, home care, clinic, etc.)
  • Scheduling or coordination experience
  • EMR experience (WellSky/Kinnser or similar)
  • Background as LPN, PTA, COTA, Medical Assistant, or similar

Responsibilities

  • Assign clinicians (RNs, therapists, aides) to all patient visits
  • Manage and maintain the master schedule in our EMR system
  • Handle call-offs, last-minute changes, and scheduling gaps in real-time
  • Ensure no missed visits and full patient coverage
  • Act as the go-to person for urgent scheduling issues
  • Resolve problems quickly, including early mornings, evenings, and occasional after-hours situations
  • Communicate schedules and updates clearly with field staff
  • Follow up on missed visits, delays, and responsiveness
  • Hold clinicians accountable for timeliness and communication
  • Escalate ongoing issues to clinical leadership
  • Track physician orders from creation to signed completion
  • Follow up on outstanding orders and ensure timely turnaround
  • Maintain organized tracking of all orders
  • Monitor completion of clinical notes for timeliness and completeness (non-clinical)
  • Follow up on missing or late documentation
  • Communicate with patients and families regarding scheduling
  • Handle rescheduling, delays, and service concerns
  • Help resolve issues while maintaining a positive experience
  • Manage workflow within our EMR (WellSky/Kinnser or similar)
  • Track key metrics such as visit completion and order turnaround
  • Identify and improve operational inefficiencies
