About The Position

The Outpatient Clinical Nurse, LPN is responsible for objectively assessing, planning, implementing, and evaluating nursing care in coordination with a multidisciplinary team to provide “patient centered” care. The Licensed Practical Nurse, under the direction of a provider, will assist in the ongoing management of the patient based on identified psycho-social needs, continuity of care, disease prevention, chronic disease management, and provides patient education to promote Health Literacy.

Requirements

Currently licensed as a Licensed Practical Nurse in the State of Nebraska or multi state licensure in a nurse licensure compact state.
Certifications in Basic Life Support (BLS) required
Skilled in practical use of electronic systems for patient care and documentation.
Strong organizational skills and ability to multi-task.
Proven ability to handle confidential information with discretion, be adaptable to various competing demands, and demonstrate the highest level of service and response.
Comply with all departmental and organizational policies and procedures in addition to regulatory requirements and participate in corporate/department safety and quality activities
Must possess the ability to read, write, and speak the English language fluently.
Utilize effective, appropriate, timely communication skills.
All applicants will comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D, which require all prospective employees to sign a declaration prior to employment regarding all arrests and convictions of child abuse or violent felonies and to comply with PL 101-630 and PL 101-637 regarding criminal records check.

Nice To Haves

Certifications in Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) preferred.

Responsibilities

Formulate clinical decisions that demonstrate critical thinking to meet the physical and psychosocial needs of the patient through collaborative practice with the healthcare team.
Completes comprehensive initial assessment and completes documentation accurately and completely in the patient record.
Demonstrates expertise in systems assessment, recognition of significant changes in patient condition, establishment of priorities, and nursing interventions.
Provides interventions as ordered by the Medical Doctor, Nurse Practitioner, Physician Assistant
Evaluates patient response to intervention and promptly notifies Medical Provider of patient condition whenever necessary.
Promotes the team concept through interpersonal relationships with patients, families, and medical staff and/or other healthcare providers to coordinate medical and nursing management of patient care.
Provides patient disease preventable vaccines before discharge.
Demonstrates respect for patients’ rights, privacy, and serve as a patient advocate.
Comply with all departmental and organizational policies and procedures in addition to regulatory requirements and participate in corporate/department safety and quality activities.
At the direction of the provider provides patient and family education to ensure adequate knowledge/awareness and recommends appropriate community resources as needed.
