About The Position

The Outpatient Clinical Nurse, LPN is responsible for objectively assessing, planning, implementing, and evaluating nursing care in coordination with a multidisciplinary team to provide “patient centered” care. The Licensed Practical Nurse, under the direction of a provider, will assist in the ongoing management of the patient based on identified psycho-social needs, continuity of care, disease prevention, chronic disease management, and provides patient education to promote Health Literacy.

Requirements

  • Currently licensed as a Licensed Practical Nurse in the State of Nebraska or multi state licensure in a nurse licensure compact state.
  • Certifications in Basic Life Support (BLS) required
  • Skilled in practical use of electronic systems for patient care and documentation.
  • Strong organizational skills and ability to multi-task.
  • Proven ability to handle confidential information with discretion, be adaptable to various competing demands, and demonstrate the highest level of service and response.
  • Comply with all departmental and organizational policies and procedures in addition to regulatory requirements and participate in corporate/department safety and quality activities
  • Must possess the ability to read, write, and speak the English language fluently.
  • Utilize effective, appropriate, timely communication skills.
  • All applicants will comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D, which require all prospective employees to sign a declaration prior to employment regarding all arrests and convictions of child abuse or violent felonies and to comply with PL 101-630 and PL 101-637 regarding criminal records check.

Nice To Haves

  • Certifications in Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) preferred.

Responsibilities

  • Demonstrate the WCHS core mission, vision and values; respecting tribal beliefs, culture, and traditions
  • Formulate clinical decisions that demonstrate critical thinking to meet the physical and psychosocial needs of the patient through collaborative practice with the healthcare team.
  • Completes comprehensive initial assessment and completes documentation accurately and completely in the patient record.
  • Demonstrates expertise in systems assessment, recognition of significant changes in patient condition, establishment of priorities, and nursing interventions.
  • Provides interventions as ordered by the Medical Doctor, Nurse Practitioner, Physician Assistant
  • Evaluates patient response to intervention and promptly notifies Medical Provider of patient condition whenever necessary.
  • Promotes the team concept through interpersonal relationships with patients, families, and medical staff and/or other healthcare providers to coordinate medical and nursing management of patient care.
  • Provides patient disease preventable vaccines before discharge.
  • Demonstrates respect for patients’ rights, privacy, and serve as a patient advocate.
  • Comply with all departmental and organizational policies and procedures in addition to regulatory requirements and participate in corporate/department safety and quality activities.
  • At the direction of the provider provides patient and family education to ensure adequate knowledge/awareness and recommends appropriate community resources as needed.
  • Other duties as assigned.
