About The Position

The Clinical Nurse Liaison at Memorial Hermann Health System is responsible for conducting clinical assessments of patient referrals and presenting this information to physicians. This role involves marketing hospital services and improving quality through collaboration with external referral sources, social workers, discharge planners, case managers, healthcare professionals, families, and patients. The position aims to enhance the utilization of clinical services offered by Memorial Hermann while ensuring high-quality patient care and exceptional experiences for the community.

Requirements

  • Graduate from an accredited school of professional nursing, vocational nursing, or licensed practical nursing.
  • Current Texas RN license required; BSN, CRRN, CNRN or CCM preferred.
  • Certificate in Basic Life Support required.
  • Minimum of three (3) years nursing experience in rehabilitation, ER, SICU/NICU nursing, or related field.
  • Experience in a similar position or Case Management strongly preferred.
  • Ability to manage multi-task, prioritize work, and pay close attention to details.
  • Excellent organizational and communication skills.
  • Clinical knowledge to assess patients for hospital programs.
  • Ability to effectively communicate complex ideas both orally and in writing.
  • Ability to work independently with little or no supervision.

Nice To Haves

  • Experience in marketing hospital services.
  • Knowledge of various data systems.

Responsibilities

  • Conduct clinical assessment of patient referrals and present assessments to physicians for admission.
  • Participate in ongoing process improvement efforts related to the admissions process.
  • Serve as a resource to directors to identify processes that hinder referral sources' ability to use Memorial Hermann as their facility of choice.
  • Demonstrate a complete understanding of programs and effectively market services.
  • Participate in hospital marketing programs, events, conferences, or in-services.
  • Develop strong working relationships with external referral sources and internal staff including Admissions, physicians, and Marketing staff.
  • Ensure safe care to patients, staff, and visitors while adhering to all policies and standards.
  • Promote individual professional growth and development by meeting requirements for continuing education and skills competency.
  • Model Memorial Hermann's service standards by providing safe, caring, personalized, and efficient experiences.
