About The Position

The position is for a full-time Registered Nurse in the Progressive Step Down unit at Carolinas Medical Center. The nurse will perform holistic assessments, develop and implement care plans, and manage patient care within their microsystem. Responsibilities include troubleshooting complex patient care issues, documenting findings, and providing education to patients and families. The nurse will also utilize information technology to access evidence-based practices and communicate patient progress to the healthcare team. The role requires daily review of patient information and collaboration with care providers to improve patient outcomes.

Requirements

  • Master's Degree in Nursing in the CNL track required.
  • Current license to practice as a Registered Nurse in the applicable state.
  • CNL certification from the Commission on Nurse Certification of the American Association of Colleges of Nursing required.
  • BLS certification required per policy guidelines.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state of practice.

Responsibilities

  • Perform or oversee the collection of holistic assessments and the development and implementation of a plan of care.
  • Carry a caseload of patients within their microsystem.
  • Oversee the management of patient care.
  • Troubleshoot complex problems in patient care and document findings in patient records.
  • Provide briefings to new care providers and review pertinent information for care across the continuum.
  • Review all patient information, including lab and other test results and progress notes in patient health records daily.
  • Develop partnerships with care providers and patients/families to set patient goals.
  • Provide or oversee education for patients and families on illness and self-care concepts.
  • Utilize information technology to obtain evidence-based practices and provide current best practice information to care providers.
  • Develop and utilize tools to evaluate, summarize, and communicate patient progress toward outcomes.
  • Review data on nurse-sensitive outcomes and incident reports to compare with system-wide and national data.
  • Seek opportunities to make improvements to reduce risk.
  • Work with team members, leaders, and physicians to develop performance improvement indicators.
  • Delegate, supervise, and evaluate nursing care given by others while retaining accountability for quality of care.
