About The Position

Duke Health Integrated Practice is an exceptional, multi-specialty physician practice encompassing 140 clinics across North Carolina. We are seeking a Clinical Nurse II to independently plan and provide professional nursing care for patients in ambulatory clinic settings in accordance with the medical and nursing plans of care and established policies and procedures. Provide nursing services to patients and families in accordance with the scope of the RN as defined by the North Carolina Board of Nursing. Provide concierge care to patients in the Primary Care Medical Home setting (Signature Care) and Health.

Requirements

  • Graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
  • All nurses hired after July 1, 2014 with less than a Baccalaureate in Nursing degree are encouraged to enroll within 2 years of RN start date and are required to complete their baccalaureate (or higher) nursing degree within 7 years of their hire date.
  • Adult Medical/Surgical, ED, Critical Care, Cardiac or previous Ambulatory Care experience within the last 5 years preferred.
  • Must have current or compact RN licensure in the state of North Carolina.
  • BLS required.

Responsibilities

  • Participate in the identification of clinical, operational, and safety related performance improvement activities; participate in daily huddles and actively engage in problem solving.
  • Plan, provide, and document professional nursing care in the ambulatory setting utilizing the nursing process and in accordance with physician orders/ established policy and procedures.
  • Perform patient intake assessment and documentation utilizing double identifiers so that patient is ready to be seen by provider on time.
  • Participate in own professional development by maintaining required competencies, assessing learning needs, and continuing education as required by the NCBON/ other Certification Agencies.
  • Maintain current knowledge of policies/procedures as set forth by DHIP/DUHS, other regulatory agencies, and those that are clinic specific.
  • Assist/chaperone provider with procedures (Pap smear, Breast/Rectal exams, Shave Biopsy, Intra-articular Injection/Aspiration, etc.).
  • Collect and analyze patient data, make recommendations for and evaluate nursing care; document and communicate nursing actions taken and patient response.
  • Promptly address staff messages, patient prescription refill requests, patient calls, prior authorizations, faxing, etc. during work shift; walk-in and telephone triage as needed.
  • Perform care tasks within nursing scope (includes, but not limited to, EKG's, ear lavage, medication administration utilizing Six Rights, Immunizations, nebulizer treatments, suture removal, wound dressings, and pulse oximetry as needed).
  • Perform Phlebotomy, Point of Care Testing, and other lab services utilizing appropriate policy and procedure guidelines.
  • Perform equipment checks/controls; maintain accurate clinic logs with required documentation.
  • Monitor inventory, clean/stock exam rooms, check for expiration dates; report any issues to clinic management.
  • PIV insertion for patient hydration and urinary catheterization as ordered by provider.
  • Maintain orderly work environment by keeping work station clean/tidy, hallways and patient care areas hazard free.
  • Educate the patient/ family/ support persons regarding medications, treatment, plan of care; document in the Electronic Health Record (EHR).
  • Delegate tasks/supervise activities of unlicensed assistive personnel; Assist other nursing personnel in the delivery of patient care; support development of other staff learners.
  • Coordinate patient movement throughout clinic to ensure smooth workflow, avoid bottlenecks, and completion of procedures in a timely fashion.
  • Provide continuous excellent customer service in a concierge care practice; explain delays, report service recovery needs to clinic management when necessary.
  • Perform other related duties incidental to the work described herein.
