About The Position

Clinical Nurse Coordinator – Specialty Pharmacy Axium Healthcare Puerto Rico, a Carelon Company, is a valued member of Elevance Health’s family of companies. As a Clinical Pharmacy servicing patients with chronic illnesses requiring complex care in Puerto Rico. We optimize patient outcomes by blending a high-touch, caring patient experience with specialized clinical knowledge, personalized total life care programs and administrative expertise. Office Location: 1001 San Roberto Street, STE 101, 401, & 501, San Juan, PR 00926 In-Office Expectation: Hybrid 1: This role requires associates to be in-office 1 - 2 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace. Please note that per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law. Schedule: Monday – Friday; 7:00 AM – 6:30 PM, specific 8-hour shifts rotate on a monthly basis. The Clinical Nurse Coordinator is responsible for collaborating with healthcare providers and/or pharmacists, as part of the health care team, to drive personalized health management and improve health outcomes for optimal consumers.

Requirements

  • Requires an ASN or ADN and a minimum of 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background.
  • Current, valid, active unrestricted RN license in Puerto Rico required.

Nice To Haves

  • BSN preferred.
  • Bilingual – Spanish and English; comfortability in both languages is strongly preferred to support main customer base completely.
  • Experience within a specialty pharmacy is strongly preferred.
  • Experience with medication management programs strongly preferred.
  • Experience in risk evaluation and mitigation strategies (REMS) and monitoring of patients in REMS programs strongly preferred.
  • Background in clinical interventions, including documentation of interventions, aligned through specialty pharmacies, including laboratory and clinical documentation/note interpretations strongly desired.
  • Ability to work with a diverse patient population, be culturally competent, and empathetic to those with health literacy deficiencies.

Responsibilities

  • Performs care management activities within the scope of licensure for members with complex and chronic care needs, particularly with the oncology and REMS programs.
  • Ensures medically appropriate, high quality, cost-effective care and appropriateness of treatment and level of care.
  • Partners with physicians and/or pharmacists to interpret appropriateness of care, intervention planning, and general clinical guidance.
  • Collaborates with providers to assess consumer needs for early identification of and proactive planning for adverse events and continuity of care.
  • Conducts clinical assessment to develop goals that address individual needs to develop and implement a care plan.
  • Monitors and evaluates effectiveness of the care management plan and modifies as necessary.
