About The Position

Under the supervision of the Director for Care Management and Coordination or his/her designee, the Care Navigator acts as a patient advocate and care coordinator to hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Care Navigator will be responsible for coordinating patient’s care within and outside of GRMC.

Requirements

• BSN or equivalent health science degree required.
• Three (3) to five (5) years of acute care hospital experience as a registered nurse or equivalent licensed healthcare professional.
• RN license or equivalent healthcare professional license required.
• Demonstrate critical thinking skills and ability to make decisions regarding medical urgency.
• Knowledge of pathophysiology of disease to anticipate medical needs.
• Excellent verbal and written communication skills.

Nice To Haves

• Critical care or emergency department experience highly desirable.
• Experience with managed care or care coordination preferred.
• ACLS certification preferred.
