This position is based in Crestview, FL, supporting Covenant Home Health Care patients and clinical operations through our partnership with Concierge Home Care. As a Clinical Manager, RN, you will play a critical leadership role in the Covenant–Concierge partnership, overseeing clinical operations, ensuring regulatory compliance, and supporting the delivery of exceptional home health care. This role blends clinical expertise, team leadership, and operational oversight, serving as a key connector between field clinicians, leadership, and quality initiatives.

Requirements

  • Unencumbered Florida Registered Nurse (RN) license (required)
  • Valid driver’s license, auto insurance, and reliable transportation (required)
  • Willingness to travel locally as needed to support clinical operations
  • Minimum of two (2) years RN experience in acute care, med/surg, ICU, or ER settings (required)
  • At least one (1) year of skilled home health experience with demonstrated proficiency in Medicare OASIS documentation (required)
  • Strong knowledge of home health regulations, clinical compliance, and quality standards

Responsibilities

  • Supervising and coordinating home health services to ensure patients receive high-quality care as prescribed by physicians
  • Providing clinical leadership and guidance to RNs and LPNs, fostering accountability, growth, and excellence in care delivery
  • Reviewing and approving clinical documentation to ensure compliance with Medicare, Federal, insurance, and company guidelines
  • Ensuring consistent adherence to agency policies, procedures, and standard operating protocols
  • Maintaining compliance with accepted professional standards and regulatory requirements
  • Staying current on all applicable CMS, Medicare, and state guidelines to support quality outcomes and survey readiness
  • Supporting field operations as needed, including occasional patient visits (compensated separately from salary)
