Signature Healthcare at Home is seeking to add a Clinical Manager (RN) for Whatcom County! This position is a hybrid role that is 50% focused on field-based clinical work and 50% based on administrative clinical work in-office. This role is responsible for admissions in the field, triage coverage, processing clinical workflow tasks in the EMR and supervises a small team of other nursing employees. The ideal candidate is someone who has home health experience as either a clinician or manager, and is eager to grow and own their own team! Duties and Responsibilities Handles the initial start-of-care process for new home health referrals by doing in-field admissions with patients or assigning them to the appropriate subordinate for completion, ensures that weekly admission goals are met. Oversees the triage coverage needs of the home health agency, including writing the on-call calendar, handling on-call rotations and assisting with clinician questions or patient needs during on-call. Processes daily workflow in the EMR (Homecare Homebase) to include new orders, reviewing the Medicare 485, reviewing of clinical staff schedules and plan of care for patients on census. Attends, runs and documents case conference meetings and organizational personnel to facilitate coordination of care. Evaluates registered nurse and licensed practical nurses’ performance and participates in the clinical onboarding and staff training. Assists the Director of Clinical Services and/or Executive Director in the planning, implementation and evaluation of in-service and continuing educational programs.

Requirements

  • Must be a registered nurse with current licensure to practice in the state of operation (Washington).
  • Previous experience in a home care setting required and two years’ experience in a managerial or supervisory capacity.
  • Proven ability to work within an interdisciplinary setting.
  • Must possess and maintain throughout employment a valid driver’s license, CPR certification, and auto insurance with the ability to drive for agency related travel (E.g., patient visits) individual’s own vehicle.
  • Strong interpersonal skills with both verbal and written communication.
