About The Position

Clinical Manager - Nursing (Home Healthcare) We are growing and looking to add a net new Clinical Manager - Nursing to our Nursing Leadership Team! The Clinical Manager is accountable for the day-to-day clinical execution, staff performance, and documentation quality of assigned home health nursing team members. This role directly supervises nursing staff and ensures care delivery, documentation, and clinical decision-making meets CareAparent standards, regulatory requirements, and productivity expectations. Reporting to the Executive Director, the RN Clinical Manager serves as the primary frontline leader for Case Managers, SOC/ROC RNs, LPNs, and other assigned staff. This role integrates people leadership, quality assurance, and operational execution to support safe, timely, and high-quality patient care across the full episode of care. This position requires a hands-on clinical leader who coaches consistently, addresses performance gaps early, identifies clinical risk, and uses data to drive accountability and improvement. Requirements

  • A valid Registered Nurse (RN) license in the state of Minnesota
  • At least FIVE (5) years of experience in home care
  • At least THREE (3) years of management experience
  • A valid Driver's License and an insured, reliable vehicle
  • Leadership and Team Management: Proven ability to lead, mentor, and manage clinical teams, fostering a culture of accountability and collaboration.
  • Clinical Expertise: Strong knowledge of home health care practices, regulations, and evidence-based care delivery standards.
  • Operational Efficiency: Experience in optimizing workflows, managing productivity, and achieving key performance indicators (KPIs).
  • Quality Assurance and Compliance: Knowledge of QAPI initiatives and the ability to ensure adherence to accreditation, regulatory, and organizational standards.
  • Effective Communication: Excellent written and verbal communication skills to clearly convey information, motivate staff, and maintain productive relationships with clients, families, and stakeholders.
  • Problem-Solving and Critical Thinking: Strong analytical and decision-making skills to address operational challenges and develop innovative solutions
  • Organizational Skills: Ability to manage multiple priorities, coordinate schedules, and maintain accurate documentation in a demanding environment.

Responsibilities

  • Directly supervise, coach, and support assigned nursing staff to ensure consistent clinical performance and quality care delivery.
  • Conduct regular field supervision and competency validation, providing clear, timely feedback.
  • Support onboarding and ongoing development of clinical staff through structured coaching and defined expectations.
  • Address performance concerns promptly using documented coaching and corrective action when required.
  • Hold staff accountable for productivity, documentation timeliness, quality outcomes, and professional conduct.
  • Ability to take ownership of work assigned and can complete work with follow through and minimal direction.
  • Reinforce CareAparent’s mission, values, and culture through daily leadership behaviors and decision-making.
  • Provide ongoing training and education to staff as needed through team meetings, workshops, skills fairs, and annual competencies.
  • Optimize clinical workflows.
  • Maintain up-to-date knowledge of COPs, state regulations, and The Joint Commission accreditation standards.
  • Ensure staff are operating and following state, CMS, and The Joint Commission regulations.
  • Conduct clinical chart audits for survey preparation.
  • Perform hands-on Quality Assurance (QA) for assigned nursing staff:
  • Chart / clinical documentation review
  • OASIS accuracy and integrity
  • Timely documentation and completion
  • Compliance with CMS, State, and accreditation requirements
  • Oversee documentation and care planning to ensure alignment with physician orders, evidence-based practice, and regulatory standards.
  • Identify documentation trends and implement targeted remediation and coaching.
  • Partner with Compliance and QAPI to support audits, corrective action plans, and survey readiness.
  • Support reduction of hospitalizations, documentation deficiencies, and billing denials through proactive QA oversight.
  • Ensure staff are trained fully in using EMR correctly and efficiently.
  • Review referrals to ensure clients are within scope of practice for delivering care.
  • Oversee the full patient episode lifecycle, including SOC, recertifications, changes in condition, and discharge.
  • Support clinical triage and escalation for high-risk patients to prevent avoidable hospitalizations.
  • Ensure care plans remain accurate, current, and clinically appropriate.
  • Participate in on-call coverage as assigned, supporting clinical decision-making during non-business hours.
  • Conduct investigations into any reported incidents, complaints, med errors, etc.
  • Monitor and manage staff productivity, clinical outcomes, and key performance indicators for assigned teams.
  • Actively manage daily workflows, including visit availability, coverage needs, and operational efficiency.
  • Make real-time adjustments to staffing and workflows to ensure productivity targets are met.
  • Collaborate with intake, scheduling, and operations teams to support timely admissions and continuity of care.
  • Provide direct patient care visits as needed to ensure safe coverage and service continuity.
  • Manage key performance metrics, including:
  • Productivity (i.e., availability and points)
  • Timely submission
  • Point of Care documentation
  • OASIS and QA findings
  • Hospitalizations and change-in-condition trends
  • Coach teams to improve performance under HHVBP (home health value-based payments) measures and quality benchmarks.
  • Use data to drive performance conversations, workflow improvements, and accountability.
  • Escalate clinical risks, staffing constraints, or capacity concerns to leadership with recommended solutions.
  • Manage episode utilization of staff and financial performance to maximize outcomes while maintaining fiscally responsible management of costs associated with client episodes of care.
  • Participate in IDT meetings and interdisciplinary collaboration.
  • Serve as a clinical resource for the nursing team.
  • Collaborate with clinical leadership team (i.e., ED, Administrator, Compliance, QAPI, etc.) regarding improvements in care and outcomes.
