About The Position

POSITION SUMMARY: The MVPP (Multi Visit Patient Program) Clinical Manager will drive the clinical services for a specialized care management program targeted to impact utilization and health outcomes for identified multi-visit patients within BMC’s Emergency Department. This work will be done collaboratively with leadership and providers from across the health system, including the Emergency Department, Population Health, Psychiatry, and Grayken Center for Addiction. This role will bring unique expertise in behavioral health, social service resources, and emergency services. This position will also engage internal and external community partners to enhance care coordination and resources available to this patient population. Responsibilities will include recruitment and supervision of BSW, MSW, and MPH students providing direct patient care and administrative support to the project. Requirements

  • Master's in Social Work (MSW)
  • Active Massachusetts LICSW (Licensed Independent Clinical Social Worker) preferred. Will consider LCSW with relevant experience and desire to work towards LICSW licensure. Will consider MSW with relevant experience who is eligible and obtains LCSW license within 6 months of hire.
  • Minimum two years of experience working in social services, healthcare, or public health setting.
  • Extensive knowledge of community resources and healthcare systems, including state agencies such as DMH, DDS and MassHealth.
  • Clinical knowledge of mental health and substance use disorders, as well as skills/passion to engage individuals with these diagnoses utilizing person-centered approaches and social work values.
  • Knowledge of homeless services and passion for serving individuals who are unhoused through a non-stigmatizing, patient-centered approach.
  • Ability to practice and teach cultural humility and desire to work with diverse, multi-cultural and multi-lingual populations and colleagues.
  • Outstanding interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with direct reports, healthcare providers, patients and families, general public, staff members, external agencies, and other organizational leaders.
  • Ability to give direct feedback and regular coaching to students; ability to mentor, guide and motivate others through demonstration of best practices, leading by example, and consistent demonstration of social work values.
  • Ability to adapt to change/s within the project strategy but also within the larger health system.
  • Exceptional organizational and time management skills; ability to multi-task and work independently, and as part of a team.
  • Excellent oral and written English communication skills.
  • Intermediate Microsoft Office suite experience (i.e. MS Word, Excel, Access, Outlook, Teams).

Nice To Haves

  • Minimum of five years of relevant work experience or equivalent is preferred.
  • At least one year of management/supervisory experience preferred.
  • Prior experience working in emergency services setting preferred.
  • Prior experience providing case management services to high risk and complex populations preferred.
  • Prior experience in homelessness services is preferred.
  • Lived experience is highly valued.
  • Preference for individual with knowledge of Boston area resources specifically.
  • Bi-lingual and/or bi-cultural clinician preferable.

Responsibilities

  • Address unmet clinical and social needs of super utilizers of BMC’s ED through direct patient engagement, care coordination, crisis planning, and resource connection.
  • Supervise, coach, mentor BSW, MSW, and MPH students in the program including recruitment, clinical supervision, training, patient facing coaching, and managing relationship with host institution, through an overall commitment to social work and public health education. May require limited clinical coverage on nights/weekends for students, shared across staff.
  • Build and leverage relationships with ED staff, clinical and non-clinical, to best meet the needs of a complex patient population including daily presence at ED huddle, bed-side engagement of patients, and representing program in various meetings.
  • Utilizing patient-centered lens and harm reduction approach, support the safety of high-risk patients through care planning, transitions of care support, and case conferencing, including ability to make clinical decisions independently.
  • Complete timely and thorough documentation in the electronic medical record and comply with all data entry, data integrity, and data tracking requirements for BMC and in compliance with MassHealth billing requirements.
  • Engage with relevant internal and external partners, including departments and programs across the health system, and local city/state agencies, shelters, etc.
  • In collaboration with program and hospital leadership, support additional program needs including administrative tasks such as data entry, aiding in program analysis, and contributing to strategy as needed. Must adhere to all of BMC’s RESPECT behavioral standards.
