Responsibilities The Clinical Manager - Hospice at the direction of the Director of Hospice, the Clinical Manager, plans for, leads, coordinates and facilitates the provision of quality care and service, including appropriate use of resources (human/supplies/equipment), for a patient at the end of life. Demonstrates commitment to hospice and palliative care, excellence in problem solving, leadership, communication, fiscal responsibility and teamwork. Provides leadership, consultation and collaboration in a dynamic, multidisciplinary environment and collaborates with administrators and other members of the health care team to design, evaluate, and improve patient care systems and processes. Requirements

  • Bachelor’s degree in professional clinical discipline (RN or MSW) from an accredited college or university, master’s degree desired. Certification in specialty preferred.
  • Two (2) year of experience in a direct supervisory role required.
  • Must have excellent interpersonal, communication and analytical skills.
  • Software skills: Electronic medical record, Microsoft Excel, Word, Outlook, PowerPoint, etc.
  • Professional licensure required for VT and NH (or compact license), or MSW required.

Nice To Haves

  • master’s degree desired.
  • Certification in specialty preferred.
  • Specialty certification in hospice required within one year of hire.

Responsibilities

  • Manages the operational, administrative, and personnel responsibilities in providing services in a 24/7 work environment by overseeing day-to-day operations.
  • Contributes to the development of annual budget; monitor patient revenues, clinical, and operational expenses and staff productivity. Reports variance to Director and takes appropriate actions to meet or exceed budget targets.
  • Monitors key performance indicators and creates an action plan to improve results.
  • Oversees case management process; strategizes to improve efficiency and eliminates barriers that impact patient and family centered care. Optimizes use of resources to meet patient needs.
  • Facilitates and monitor activities related to the delivery of patient care, assuring evidence-based practice and minimum clinical practice standards are achieved in the clinical area.
  • Administers clinical care to patients, as needed, to maintain clinical skills, to assess particular patient requirements or as support to patient care staff.
  • Oversees, facilitates, and monitors all aspects of the hospice personnel; including orientation of new staff, development and education of clinicians, and performance management
  • Supports clinicians/patients in the field, as needed.
