About The Position

The Clinical Manager for Home Health develops, plans, implements, analyzes, and organizes clinical operations for a specific location managed. This role includes conducting and delegating the assessment and reassessment of patients, updating care plans, and interpreting patient needs while adhering to company, physician, and health facility procedures and policies. The manager is responsible for overseeing the delivery of care to all patients served by the location, managing the assignment of caregivers, and ensuring compliance with regulatory agencies and third-party payer requirements. The Clinical Manager also participates in sales and marketing initiatives, supervises clinical employees, and handles employee corrective actions and discipline issues. Additionally, the role involves assessing staff education needs, conducting regular staff education, and maintaining relationships with referral community sources.

Requirements

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR certification.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one year of management experience in home care, hospice, or equivalent environment.

Responsibilities

  • Develops, plans, implements, analyzes, and organizes clinical operations for a specific location.
  • Conducts and delegates patient assessments and reassessments, updating care plans as necessary.
  • Manages the assignment of caregivers and oversees the delivery of care to all patients.
  • Receives case referrals and determines home health or hospice needs.
  • Instructs and guides clinicians to promote effective performance and quality home care services.
  • Monitors cases to ensure documentation compliance with regulatory agencies.
  • Coordinates communication between team members, attending physicians, and caregivers.
  • Works with the Branch Director and Finance Department to establish revenue and budget goals.
  • Participates in sales and marketing initiatives.
  • Supervises all clinical employees and carries out supervisory responsibilities.
  • Handles employee corrective actions and discipline issues.
  • Participates in the interviewing, hiring, training, and development of direct care clinicians.
  • Evaluates staff performance and recommends in-service education programs.
  • Analyzes situations and identifies problems using Performance Improvement principles.
  • Responsible for QA/PI activities and data tracking for performance review.
  • Provides direct patient care in emergencies and acts as Branch Director in their absence.
  • Maintains relationships with referral community sources and participates in professional organizations.
