Job Summary: Reporting to the Director of Nursing, the Clinical Manager for Field Nurse Assessors will oversee the clinical quality, compliance, and performance of nurse assessors conducting in-home evaluations. This role ensures high standards of patient care, accurate clinical assessments, regulatory compliance, and effective coordination between clinical staff, patients, and administrative teams. The Clinical Manager provides leadership, training, and quality oversight to support safe, patient-centered home care services.

Requirements

  • Active NYS RN license in good standing
  • Minimum 3–5 years of clinical nursing experience
  • Experience in home care, home health, or community-based nursing
  • Prior leadership, supervisory, or management experience
  • Strong knowledge of clinical assessment standards and documentation
  • Strong clinical judgment and decision-making skills
  • Excellent communication and leadership abilities
  • Organizational and time-management skills
  • Ability to work independently and manage remote teams
  • Proficiency with electronic health records (EHR) and clinical software
  • Ability to travel to patient homes and staff locations as needed
  • Ability to sit, stand, and use computer systems for extended periods

Nice To Haves

  • Bachelor of Science in Nursing (BSN) or higher
  • Experience managing field-based or remote clinical teams
  • Knowledge of Medicare/Medicaid home care regulations
  • Case management or utilization review experience

Responsibilities

  • Supervise and support field nurse assessors conducting in-home patient assessments
  • Review and validate clinical assessments, care recommendations, and documentation
  • Ensure compliance with state, federal, and payer regulations (e.g., Medicare, Medicaid, insurance guidelines)
  • Monitor clinical outcomes and implement quality improvement initiatives
  • Address clinical escalations, complex cases, and patient safety concerns
  • Hire, onboard, train, and mentor field nurse assessors
  • Conduct performance evaluations and provide coaching and corrective action as needed
  • Ensure clinical competency and continuing education compliance
  • Foster a culture of accountability, professionalism, and patient-centered care
  • Coordinate field staff scheduling and coverage to meet service demands
  • Collaborate with intake, care coordination, and administrative teams
  • Ensure timely completion and submission of assessments and reports
  • Participate in audits, surveys, and accreditation processes
  • Serve as a clinical resource for patients, families, and caregivers
  • Support resolution of patient concerns and complaints
  • Promote compassionate, respectful, and culturally sensitive care
  • Maintain adherence to DOH, Joint Commission and other agency policies and procedures
  • Identify and mitigate clinical risks
  • Ensure accurate, complete, and timely clinical documentation
