About The Position

As a Clinical Liaison at Select Specialty Hospital, you will advocate for referred patients during the pre-admission process by collaborating with clinical teams to assess patient needs. Your role will involve serving as a resource for healthcare professionals and fostering relationships within medical facilities. Utilizing your clinical expertise and problem-solving skills, you will coordinate care plans with physicians and families, facilitate patient transfers, and address barriers to accessing specialized post-ICU care. This position is full-time, Monday to Friday from 8 AM to 5 PM, and offers a competitive salary along with uncapped monthly bonus incentives and customizable health insurance packages.

Requirements

  • Registered Nurse (RN) or Respiratory Therapist (RT) License.
  • Consideration for other designations like LPN, PT, OT upon further review.
  • Ability to complete clinical assessments of medically complex patients.

Responsibilities

  • Advocate for referred patients in the pre-admission process by collaborating with clinical teams.
  • Serve as a resource for healthcare professionals and foster relationships within medical facilities.
  • Coordinate care plans with physicians and families using clinical expertise and problem-solving skills.
  • Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
  • Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay.
  • Perform clinical assessments of referred patients and counsel families to assist in care decisions.
  • Partner with hospital case managers and physicians to create smooth pre-admissions and transfer processes.
