About The Position

The Clinical Liaison at Ascend Healthcare Management is responsible for establishing and maintaining relationships with healthcare providers and facilities to facilitate patient admissions and ensure continuity of care. Requirements

• Bachelor's degree in nursing, social work, or a related field.
• Current nursing license or relevant certification in the state of Arkansas.
• Minimum of 2 years of experience in a clinical setting, preferably in a liaison or case management role.
• Strong communication and interpersonal skills to build relationships with healthcare providers and patients.
• Ability to work independently and manage multiple priorities effectively.

Nice To Haves

• Experience in home health or hospice care settings.
• Knowledge of healthcare regulations and compliance standards.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Develop and maintain relationships with referral sources, including hospitals, physicians, and other healthcare providers.
• Conduct assessments of patients to determine appropriate care needs and services.
• Coordinate patient admissions and transitions of care between facilities and home health services.
• Provide education and support to patients and families regarding available services and resources.
• Collaborate with clinical teams to ensure comprehensive care planning and execution.
