About The Position

A Path of Care Home Health is seeking a full-time Clinical Liaison to support the growth and business development of our agency. This role combines clinical expertise with relationship management to ensure that patients transitioning from a hospital to home-based care do so safely and without interruption in their treatment.

Requirements

• Licensure : Most positions require a current clinical license, typically as a Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) , though some roles may accept Respiratory Therapists (RRT) or social workers.
• Experience : Employers usually look for at least 1-3 years of experience in home health, hospice, or acute care settings.
• Essential Skills : Strong communication, empathy for end-of-life care, organizational ability, and familiarity with Electronic Medical Records (EMR) and CRM software.

Responsibilities

• Patient Assessments : Conducting clinical evaluations of referred patients to determine if they meet the specific eligibility criteria for home health or hospice services.
• Care Coordination : Collaborating with hospital case managers, discharge planners, and physicians to facilitate a seamless "handoff".
• Relationship Building : Maintaining professional partnerships with referral sources (doctors and social workers) to ensure the agency remains a top choice for patient placement.
• Patient Advocacy & Education : Meeting with patients and their families to explain the benefits of home care or hospice, answer questions about insurance coverage, and manage expectations for the transition.
• Marketing & Growth : Tracking referral activity and implementing outreach strategies to help grow the agency's patient census.
