About The Position

The Clinical Liaison at Pate Rehabilitation is responsible for generating patient referrals and coordinating the pre-screening and admission processes. This role serves as the primary contact for families and patients, ensuring a streamlined admission process while promoting positive community relations and adherence to the organization's philosophy.

Requirements

• Degree in Nursing or related therapy field (SLP, OT, PT, LCSW).
• Unrestricted license to practice in their home state.
• Minimum of five years' experience in health care accreditation, preferably in CARF accredited operations.
• Minimum of five years' experience in quality, data management, clinical informatics, or electronic medical records (EMR).
• Effective interpersonal, verbal, and written communication skills.

Responsibilities

• Generate patient referrals and coordinate pre-screens and admissions.
• Serve as the primary contact for families and patients.
• Partner with the admissions team, program manager, and other departments to streamline admission processes.
• Promote the Rehab Without Walls philosophy to ensure patient satisfaction.
• Provide referral sources with progress reports and information in compliance with HIPAA guidelines.
• Support the development of pre-screen and admissions resources and processes.
• Identify special needs of potential patients and schedule pre-admission meetings.
• Coordinate the admission process with medical staff and provide feedback to the Admissions Coordinator.
• Educate the community about brain injury and advocate for necessary services during the admissions process.
• Create and maintain information for scheduling pre-admission evaluations.
• Contact patients/families to schedule pre-admission evaluations within 24 hours of referral.
• Travel as needed for the role.
