CLINICAL LIAISON - RN
About The Position
To provide clinical assessment services to referral sources, families and patients to determine the appropriate level of care needed for potential admission to Lakeview continuum of services. Manages assigned territory for referral development. Participates in the development of the annual strategic plan and integrates to daily activities. Lakeview Specialty Hospital and Rehab is a Joint Commission Accredited and licensed long term acute care hospital (LTACH) and Inpatient Rehabilitation Hospital (IRF) that has delivered 30 years of innovative programs and extensive services with expertise in medically complex care, neurobehavioral health, rehabilitation, and community supportive living. Requirements
- Minimum of Bachelor Degree or consideration of work history will be evaluated
- Valid driver’s license with clean driving record
Nice To Haves
- Registered Nurse (VERY HIGHLY PREFERRED)
- Master’s Degree in health care field
- Minimum of two years healthcare experience preferred
- Marketing/Sales experience HIGHLY preferred
- Knowledge of rehabilitation and/or LTAC a plus
Responsibilities
- Provide clinical assessment services to referral sources, families and patients to determine the appropriate level of care needed for potential admission to Lakeview continuum of services.
- Manage assigned territory for referral development.
- Participate in the development of the annual strategic plan and integrate to daily activities.
- Complete onsite clinical evaluations for patients referred for services at Lakeview.
- Effectively manage primary assigned territory in an efficient and cost-effective manner.
- Participate in active referral development.
- Complete and regularly update competitive analysis on assigned market.
- Adhere to facility policies and procedures including assisting in maintaining JCAHO standards.
