About The Position

The Clinical Liaison, RN at Faith Presbyterian Hospice plays a crucial role in ensuring that patients and their families receive the highest quality of care during their hospice journey. This position is responsible for facilitating access to hospice and palliative care services, ensuring that patients' needs are met with compassion and professionalism. The Clinical Liaison will model a service-oriented attitude, taking ownership of problem-solving and demonstrating initiative in improving patient care processes. The role requires a strong commitment to safety, as the Clinical Liaison must remain vigilant in identifying and reporting any safety hazards that may affect residents, patients, visitors, or co-workers. In this position, the Clinical Liaison will respond to referrals promptly, utilizing effective listening and assessment skills to understand the unique needs of each patient and their family. The Liaison will work closely with Social Services, Case Management, physicians, and hospital nursing staff to coordinate care for referred patients and those already in hospice care. This includes conducting initial assessments, collaborating on treatment plans, and providing education about available programs and services. The Clinical Liaison will also support patients and families in navigating end-of-life decisions, ensuring that communication is clear and supportive. Documentation is a critical aspect of this role, as the Clinical Liaison must maintain accurate records in compliance with hospital and hospice policies. This includes documenting patient eligibility for hospice services, coordinating discharge planning, and participating in interdisciplinary team meetings. The Clinical Liaison will also assist in pain and symptom management by providing feedback to the Hospice Medical Director and hospital physicians regarding patient responses to treatment. Overall, this position is integral to the hospice team, contributing to the development of care plans and ensuring that patients receive the support they need during a challenging time.
