About The Position

As a Clinical Liaison (RN/RRT) at Select Specialty Hospital, you will advocate for referred patients during the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals, fostering relationships within medical facilities. Utilizing your clinical expertise and problem-solving skills, you will coordinate care plans with physicians and families, facilitate patient transfers, and address barriers to accessing specialized post-ICU care.

Requirements

  • Current and valid state licensure or eligibility for state licensure as a Registered Nurse or Respiratory Therapist.
  • Two years of related experience working in a clinical healthcare setting or previous experience in a clinical liaison role.
  • Demonstrated competency with the preadmission process, including assessments and relationship building in a rehabilitation or post-acute care setting.
  • Proven track record in sales and marketing with at least one year of experience in a related position.
  • Demonstrated computer proficiency.
  • Ability to exercise sound judgment in handling the professional/confidential nature of health care sales.

Responsibilities

  • Advocate for referred patients in the pre-admission process.
  • Collaborate with clinical teams to assess patient needs.
  • Serve as a resource for healthcare professionals.
  • Coordinate care plans with physicians and families.
  • Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
  • Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay.
  • Perform clinical assessments of referred patients.
  • Counsel with patients' families to assist them in making care decisions.
  • Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process.
