About The Position

Emmanuel Hospice is seeking a compassionate and skilled Clinical Liaison to serve as the vital connection between hospital systems, patients, families, and our hospice care team. This role blends clinical expertise with relationship-building to ensure timely, compassionate, and clinically appropriate referrals. As a member of our Business Development team, you will educate healthcare providers about hospice services, assess patient eligibility, and facilitate smooth transitions into hospice care.

Requirements

  • Valid Registered Nurse license in the State of Michigan (Associate Degree required; BSN preferred).
  • Minimum 2+ years of acute care experience; 1 year of home care or hospice experience preferred.
  • Demonstrated success in building strong partnerships with healthcare providers and families.
  • Proven experience in referral development and relationship building with community resources.
  • Proficiency in Microsoft Office, presentations, database tracking/reporting, and EMR systems.
  • Excellent oral communication skills; self-directed with the ability to work independently.
  • Ability to push, pull, move, and/or lift a minimum of 50 pounds

Responsibilities

  • Act as the primary liaison between hospital and hospice teams, strengthening partnerships and supporting referral sources.
  • Collaborate with care managers, social workers, and interdisciplinary teams to identify hospice candidates and support discharge planning.
  • Provide patient and family education on hospice services, expectations, and next steps.
  • Identify and resolve barriers to discharge (family readiness, transportation, DME delays, medication issues).
  • Coordinate placement needs and confirm appropriate levels of care (home, facility, respite, GIP-to-home).
  • Review eligibility documentation with providers and hospice Medical Director to ensure compliance.
  • Communicate real-time updates to Admissions, Scheduling, and Clinical Leadership.
  • Ensure DME and comfort medications are ordered and delivered prior to discharge.
  • Serve as the primary contact for hospice patients presenting to the ED or admitted to the hospital.
  • Maintain accurate documentation in EMR systems for all communications, referrals, and admissions.
  • Partner with Business Development and Clinical Leadership to identify referral trends and service needs.
  • Uphold compliance with Emmanuel Hospice policies and all Federal, State, and local regulations.
